Key Responsibilities

• Conduct thorough assessments of patients’ health status, including detailed history evaluations and physical examinations.
• Diagnose acute and chronic health conditions and develop tailored, evidence-based treatment plans in tandem with other healthcare professionals.
• Perform and analyze diagnostic tests as necessary, accurately prescribing appropriate medications and therapies to meet patient needs.
• Educate patients and their families on chronic disease management, symptom monitoring, medication compliance, and post-therapy care to facilitate effective health management.
• Engage in regular interdisciplinary team meetings to discuss patient progress and promote optimal care strategies.
• Document patient interactions meticulously in electronic medical records to maintain compliance with healthcare regulations and improve care coordination.
• Participate in and contribute to quality improvement initiatives that enhance patient care delivery and clinical outcomes.
• Maintain current knowledge of healthcare trends, legislative changes, and innovative practices in nursing and patient care.
