Position Overview

Amarante Medical Center is actively seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team in Atlanta, Georgia. Work Environment

The successful candidate will work onsite at our assisted living facility, which serves a diverse population of residents. Key Responsibilities

• Perform thorough health assessments and screenings, ensuring each individual receives personalized care tailored to their specific health needs. This entails reviewing medical histories, conducting physical exams, and ordering diagnostic tests as necessary.
• Diagnose both acute and chronic medical conditions, and develop effective treatment plans that include preventive care and health promotion strategies.
• Work closely with community organizations to design and implement targeted health education programs aimed at addressing prevalent health issues within the population.
• Provide culturally competent care that respects diverse backgrounds and advocates for each patient’s rights and access to necessary services.
• Ensure smooth continuity of care by coordinating referrals to specialists and arranging comprehensive follow-up care.
• Maintain accurate medical records in compliance with state regulations and organizational standards to ensure timely access to patient information.
• Engage in community outreach by attending health fairs and local events to offer services like health screenings, thereby increasing awareness of available health resources.
• Stay informed about emerging community health trends and evidence-based practices to continually enhance service delivery.

Patient Population / Client Focus

Our FNP will primarily focus on serving vulnerable populations, particularly those in assisted living situations, who often face barriers to healthcare access. You will be integral in building trusting relationships with patients and families, helping them navigate their healthcare journeys while promoting wellness and preventive care strategies.

Qualifications

• Must possess current certification as a Nurse Practitioner, along with an active state nursing license in Georgia.
• A Master's degree in nursing from an accredited program is required.
• Demonstrated commitment to community health and the principles of health equity.
• Exemplary interpersonal and communication skills are essential, with a proven ability to connect effectively with diverse populations.

Preferred Experience

• A minimum of 3-5 years of clinical experience is preferred, especially in community health or public health settings.

• Familiarity with electronic health record (EHR) systems and maintaining compliance with healthcare regulations is beneficial.
