Work Environment

This position is based in Kirkville, NY, where you will work onsite in a home care setting. You will be an essential member of our healthcare team, which includes physicians, nurse practitioners, physician assistants, medical assistants, and care managers. Key Responsibilities

• Provide exceptional primary care by conducting thorough assessments, diagnoses, and developing personalized treatment plans for patients.
• Manage office visits for routine and acute healthcare issues, tailoring approaches to individual patient needs and conditions.
• Conduct Annual Wellness Visits and comprehensive health risk assessments with a holistic perspective, documenting findings accurately to ensure continuity of care.
• Facilitate care coordination by collaborating with other healthcare providers, specialists, and community agencies to streamline patient care and ensure access to necessary resources.
• Lead population health initiatives within the care team, advocating for essential health screenings and preventative measures, such as breast cancer screenings every two years for eligible patients.
• Assist with phone triage and outreach efforts, providing timely and effective patient communication regarding health concerns and follow-up needs.
• Educate patients on their health conditions, emphasize the importance of adherence to care plans, and provide guidance on treatment options.
• Engage in promotional activities aimed at enhancing community knowledge about our services and strengthening patient trust.
• Conduct home visits as needed to address patients who may require additional support or have barriers to accessing care in the clinic.
• Report directly to the Medical Director while working closely with operational leadership to ensure alignment with organizational goals.

Patient Population / Client Focus

You will primarily serve older adults enrolled in Medicare, focusing on maintaining their health, preventing hospital admissions, and improving their quality of life. Your work will emphasize the importance of personalized care, ensuring that your patients feel valued, heard, and supported in their health journeys. This role allows for direct engagement with your patient population, fostering trust and understanding.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing medications.
• Clinical experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and conducting health risk assessments.
• Strong interpersonal skills with a passion for teamwork and collaboration.
• A commitment to innovative practices in population health management.

Preferred Experience

• Bilingual proficiency in relevant languages is advantageous and can enhance patient communication and inclusion.
• Experience working with diverse populations and understanding their unique healthcare needs.

Compensation and Benefits

The annual salary for this full-time Nurse Practitioner position ranges from $180,000 to $200,000, depending on experience and qualifications. We offer a competitive benefits package, including:
