Key Responsibilities

• Conduct thorough assessments and establish diagnoses for acute and chronic health conditions, utilizing advanced clinical judgment and evidence-based practices.
• Develop and execute tailored treatment plans that encompass pharmacological measures, therapeutic interventions, and lifestyle modifications, while ordering appropriate diagnostic tests to monitor patient progress.
• Prescribe medications, ensuring adherence to safe and effective protocols, and evaluate patient responses to treatment to optimize care strategies.
• Provide education to patients and families on disease prevention, self-management techniques, and wellness promotion, fostering health literacy and empowering patients to take charge of their health.
• Collaborate with a multidisciplinary team to coordinate comprehensive patient care, ensuring seamless transitions and continuity of care across various health services.
• Maintain accurate and timely medical records, ensuring compliance with regulatory requirements and best practices in documentation.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner from a recognized body.
• Active nursing license in California.

Compensation and Benefits
