The Vitality Plus Care Clinic is seeking a dedicated and compassionate Nurse Practitioner to join our team in Rancho Cucamonga, California. As a key member of our healthcare team, you will be responsible for delivering quality, holistic care to individuals and families. In this full-time onsite role, you will work directly within the community, conducting house calls and providing in-person care. The typical work schedule spans from 9:00 a.m. to 6:00 p.m., with flexible hours to accommodate evening clinics until 8:00 p.m. Participation in community outreach events on weekends is essential to help extend our impact. You will work alongside a collaborative interdisciplinary team, with an average commitment of around 40 hours per week to ensure each patient receives comprehensive care.
You will primarily serve diverse populations, including underserved and vulnerable communities in Rancho Cucamonga. Your mission is to provide culturally sensitive care, ensuring all patients feel valued, respected, and supported in their healthcare journey.
