Work Environment

This role is performed onsite within a home care setting, providing a dynamic work atmosphere that requires both clinical expertise and a deep understanding of community health challenges. You will collaborate closely with a multidisciplinary team, including healthcare providers, social workers, and community organizations, to create an inclusive and supportive environment for both patients and staff. Your work schedule will generally be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and community events that may extend into the weekend.

Key Responsibilities

• Perform thorough health assessments and screenings for individuals and families in the community, ensuring tailored care strategies that meet their unique needs.
• Diagnose and treat acute and chronic medical conditions, with a strong emphasis on preventive care and health promotion initiatives.
• Engage with local community organizations to spearhead and implement impactful health education programs that foster awareness and empower residents.
• Deliver culturally competent care that respects and values diverse backgrounds, advocating for the rights and needs of each patient.
• Manage care transitions and coordinate referrals to specialists, facilitating follow-up care to guarantee comprehensive patient management.

Qualifications

• A Master’s degree in Nursing from an accredited institution is required.
