Key Responsibilities

• Conduct comprehensive assessments and diagnoses for residents suffering from acute and chronic pain conditions, evaluating both physical and psychological aspects of their health.
• Develop and implement individualized, evidence-based treatment plans in coordination with physicians and other healthcare professionals, including non-pharmacological interventions like physical therapy and psychological support.
• Order, interpret, and manage results from diagnostic tests such as imaging studies or laboratory tests, ensuring timely and appropriate adaptations to treatment plans.
• Prescribe medications and therapies tailored to each patient’s clinical needs, monitoring their effectiveness and potential side effects closely.
• Conduct patient and family education sessions focusing on pain management strategies, medication management, and post-discharge care to enhance self-care and minimize hospital readmissions.
• Engage actively in interdisciplinary rounds to share insights and discuss patient care strategies, thereby fostering an integrated approach to treatment.
• Maintain meticulous electronic medical records (EMRs) to ensure accurate documentation of patient interactions and progress, contributing to continuous quality improvement initiatives within the center.
• Stay abreast of advancements in pain management practices and regulations to provide cutting-edge care supported by the latest evidence.

Patient Population / Client Focus

In this role, you will primarily serve a diverse population of residents within the assisted living facility, many of whom experience complex pain syndromes resulting from chronic illnesses, post-surgical recovery, or degenerative conditions. Your ability to connect with patients, understand their unique challenges, and provide tailored pain management solutions is vital to enhancing their comfort and functionality.

Qualifications

• A current certification as a Nurse Practitioner (NP), with special focus or experience in pain management preferred.
• An active nursing license in the state of New Jersey.
• A Master's degree in Nursing from an accredited program.
• At least 3 years of clinical experience as a Nurse Practitioner within a hospital or assisted living setting, ideally focused on pain management.
• Demonstrated ability to communicate effectively with diverse healthcare teams and exhibit strong collaborative skills.
• Proven critical thinking and problem-solving capabilities, particularly in complex case scenarios.

Preferred Experience

• Prior experience working directly in pain management or a similar specialty helps ensure a smooth transition into this dedicated role.
