Key Responsibilities

As a Nurse Practitioner at our Cardiology Clinic, your daily responsibilities will include:

• Conducting thorough patient assessments by obtaining detailed histories and performing both comprehensive and targeted physical examinations.
• Ordering, interpreting diagnostic tests, and formulating management plans for chronic and acute conditions, contributing significantly to patient care pathways.
• Independently managing a specific subset of patients while collaborating with physicians and nursing staff to optimize treatment strategies.
• Reviewing and documenting findings from internal and external medical records to ensure continuity of care.
• Designing and implementing goal-directed care plans that include health maintenance and appropriate nursing interventions, in line with healthcare protocols.
• Actively engaging in patient and caregiver education about disease management and lifestyle modifications to enhance patient self-management.
• Coordinating hospital admissions and discharges in collaboration with inpatient care teams to ensure seamless patient transitions.
• Managing patient inquiries via phone, working closely with registered nurses to address their needs and coordinating with referring providers for follow-up.
• Collaborating with a range of specialists, including Social Workers and Nutritionists, to address multifaceted patient care needs.
• Documenting all patient encounters accurately in the electronic medical records (EMR) system.
• Participating in clinical research protocols, reviewing findings, and integrating evidence-based practice into care.
• Upholding the standards of nursing practice by demonstrating accountability and responsibility for patient care delivery.
• Contributing to a therapeutic and consumer-focused environment through participation in quality improvement initiatives and safety protocols.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with patients suffering from a variety of cardiovascular diseases, including heart failure, pulmonary arterial hypertension, and those undergoing heart transplant evaluations. Experience in these specific areas enhances your ability to provide specialized care, tailored to the needs of our patient population.

Qualifications

The ideal candidate for this position will possess:

• A Master's degree in Nursing as a Nurse Practitioner with a focus on adult care.
• A current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Certification from a recognized NP board.
• Valid BCLS, ACLS, DEA, and NPI certifications, ensuring competency in emergency and acute care procedures.
• Proficiency in electronic medical record systems and standard office software, enhancing your ability to document and analyze patient data effectively.

Preferred Experience

While a minimum of two years' experience in a hospital setting is required, preference will be given to candidates with direct clinical experience in cardiology, particularly with populations facing advanced heart failure and those receiving complex therapies. Familiarity with outpatient intravenous and mechanical circulatory support therapies is also highly desirable.

Compensation and Benefits
