Work Environment

This role is primarily onsite, focusing on concierge medicine and house calls, allowing you to provide care in a comfortable and familiar setting for your patients. Your work schedule will typically be Monday to Friday, from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also be required to participate in community outreach events or health fairs during weekends, ensuring that our health services reach those who need them most. You can expect an average workweek of approximately 40 hours.

Key Responsibilities

• Perform detailed health assessments and screenings for a diverse range of patients, ensuring that their unique healthcare needs are met.
• Diagnose and manage acute and chronic medical conditions, delivering preventive care and promoting health education tailored to individual and community needs.
• Collaborate with various community organizations to create and implement impactful health education programs that equip individuals with knowledge and resources to improve their health.
• Advocate for patient rights and equitable access to healthcare, demonstrating cultural sensitivity to the backgrounds and circumstances of your patients.
• Coordinate referrals and follow-up care to ensure continuity and quality of care for patients, tracking progress through comprehensive records.
• Maintain precise medical documentation in compliance with healthcare regulations and organizational standards.
• Engage in community outreach activities, participating in health fairs and workshops to raise awareness and provide essential health screenings.
• Stay informed about new developments in community health trends, evidence-based practices, and relevant policies to enhance patient care.

Patient Population / Client Focus

Your practice will primarily serve individuals and families in Glenfield, many of whom may be from underserved and vulnerable populations. You will work to understand the particular socio-economic factors impacting their health and develop tailored care plans that address not just physical health, but social determinants of health as well. This role requires a deep commitment to social justice and health equity.

Qualifications

• A current and valid Nurse Practitioner certification.
• An active nursing license in the state of New York.
• A Bachelor's degree in nursing, ideally supplemented by a Master's degree from an accredited program.
• A minimum of 3 years of clinical experience, with a strong preference for experience in community or public health settings.
• Exceptional interpersonal and communication skills, enabling effective collaboration with a diverse patient population.
• A strong commitment to advocating for health equity and addressing the needs of vulnerable communities.

Preferred Experience

• Prior experience in concierge medicine or house call practice is highly desirable.

Compensation and Benefits
