Work Environment

As a Nurse Practitioner with Pulse Life Health Care Services, you will work onsite, primarily visiting patients in their homes within Sacramento. Your work schedule will typically be Monday to Friday from 9:00 a.m. to 6:00 p.m., with occasional extended hours for evening clinics until 8:00 p.m. Additionally, participation in community outreach events and health fairs during weekends may be required, providing further opportunities to engage with the community you serve.

Key Responsibilities

• Conduct in-depth health assessments during home visits to identify patients' medical, emotional, and social needs.
• Diagnose and effectively manage both acute and chronic health conditions, while emphasizing preventive care and health promotion strategies.
• Perform routine screenings and health education initiatives tailored to the community's unique demographics and health challenges.
• Collaborate with local health organizations and stakeholders to co-create and deliver impactful health education programs that foster better health literacy.
• Provide culturally competent care, ensuring that patients understand their rights and have access to the available healthcare services.
• Coordinate all necessary referrals and follow-up care to guarantee continuity and comprehensiveness in patient health management.
• Maintain meticulous medical records that comply with all regulatory standards, ensuring refined and safe healthcare delivery.
• Stay informed on pertinent community health trends, evidence-based practices, and relevant policies to continually enhance service delivery.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of California.
• A Master's degree in Nursing from an accredited institution is required.

Compensation and Benefits

• This role offers a competitive annual salary between $150,000 and $180,000, commensurate with experience and qualifications.
