Work Environment

The Nurse Practitioner will operate primarily onsite at patients' residences across Eugene, Oregon. Key Responsibilities

• Conduct thorough assessments and physical examinations to diagnose acute and chronic medical conditions.
• Develop, implement, and monitor comprehensive treatment plans tailored to the individual needs of each patient.
• Order and interpret diagnostic tests while prescribing appropriate medications and therapies.
• Provide education and support to patients and their families on disease prevention, medication management, and health promotion strategies.
• Collaborate extensively with a multidisciplinary healthcare team to coordinate seamless patient care and follow-up.
• Maintain accurate and detailed patient records following regulatory guidelines and organizational policies.
• Engage in quality improvement initiatives to enhance practice and patient outcomes.
• Continuously update your knowledge of healthcare practices, policies, and regulations relevant to home care.

Patient Population / Client Focus

This position emphasizes care for a diverse patient population, often including seniors, individuals with chronic illnesses, and patients recovering from surgery or hospitalization. You will have the opportunity to manage complex cases and establish trusting relationships that facilitate communication and adherence to treatment plans, ultimately improving the quality of life for patients and their families.

Qualifications

• Current certification as a Nurse Practitioner from an accredited body, recognized in the state of Oregon.
• Active Oregon nursing license to provide practice as a Nurse Practitioner.
• A Master’s degree in Nursing from an accredited program.
• Minimum of three years of clinical experience in a relevant specialty, with a focus on home care or community health desirable.
• Exceptional communication skills and the ability to work collaboratively in a team-oriented environment.
• Strong analytical abilities and problem-solving skills to assess patient needs effectively.

Preferred Experience

While a minimum of three years of experience is mandatory, candidates with additional years in home care or geriatric nursing, as well as experience in telemedicine and electronic health record systems, will be given preference. Experience in participating in research or clinical trials is also beneficial.

Compensation and Benefits

The annual compensation for this full-time position ranges from USD 110,000 to 125,000, commensurate with experience and qualifications.
