Work Environment

This position is based onsite at our assisted living facility, where you will work closely with a dedicated team of healthcare professionals. The facility offers a comfortable and engaging environment for both staff and residents, ensuring that you can focus on providing outstanding patient care. Work hours are Monday through Friday, from 9 AM to 5 PM, promoting work-life balance while you contribute to the health and well-being of our residents.

Key Responsibilities

• Provide comprehensive and empathetic care to residents, ensuring that each individual receives tailored attention that meets their specific health needs.
• Diagnose and manage a wide array of both acute and chronic health issues, utilizing evidence-based practices to inform treatment plans.
• Emphasize preventive care strategies, wellness coaching, and patient education to empower residents in managing their health.
• Collaborate within a multidisciplinary team, communicating effectively with physicians, behavioral health specialists, and care coordinators to ensure holistic patient care.
• Conduct routine physical exams, wellness visits, and follow-up care, meticulously documenting findings and treatment outcomes using our user-friendly electronic medical records (EMR) system.
• Engage in quality improvement initiatives and participate in population health programs designed to enhance community wellness.

Patient Population / Client Focus

You will primarily care for seniors and individuals with disabilities living in the assisted living facility. This patient population often presents with complex health needs, requiring an individualized and multidisciplinary approach. Your ability to build strong relationships with residents will be essential in ensuring their trust and adherence to treatment plans.

Qualifications

• A Master’s or Doctoral degree from an accredited Nurse Practitioner program is required.
• Possession of a valid national certification (Family Nurse Practitioner preferred) as well as a current or pending Florida Nurse Practitioner license.
• A strong commitment to culturally sensitive, whole-person care is essential.
• Confidence in working independently with the availability of physician support as needed.
• Excellent communication skills are crucial, along with a dedication to fostering enduring patient-provider relationships.

Preferred Experience

• At least 2 years of experience in a primary care or family medicine setting, though new graduates with relevant training and a passion for care are encouraged to apply.
• Experience in geriatric care or working within assisted living environments is a plus.

Compensation and Benefits

• Competitive salary range of $180,000.00 - $200,000 annually, commensurate with experience, along with productivity and quality performance bonuses.
