Work Environment

This role is based onsite at a local school in Largo, Florida, where you will become an integral part of the educational community. You will work Monday through Friday, from 9 AM to 5 PM, ensuring a balanced lifestyle that allows time for work and personal commitments. Our facilities are equipped with modern technology, including an electronic medical record (EMR) system that streamlines patient documentation and care coordination.

Key Responsibilities

• Conduct thorough patient assessments, including wellness visits and follow-up appointments for both chronic and acute health conditions.
• Utilize evidence-based practices to diagnose and treat various illnesses, ensuring patients receive optimal care tailored to their needs.
• Coordinate care by collaborating with physicians, specialists, behavioral health professionals, and social services to create comprehensive care plans.
• Provide education on preventive health measures and engage patients in developing wellness plans to achieve their personal health goals.
• Perform routine medical procedures appropriate for the primary care setting, utilizing your clinical skills to enhance patient outcomes.
• Leverage data from our advanced EMR system to track patient progress, identify trends, and enhance continuity of care.
• Contribute positively to population health initiatives and value-based care models, ensuring equitable access to healthcare resources.

Patient Population / Client Focus

The Family Nurse Practitioner will primarily serve students and their families, addressing a wide range of health issues from acute illnesses to chronic disease management. You will have the opportunity to impact community health directly, advocating for preventative care and education. This role emphasizes the importance of building lasting relationships with patients, fostering trust and continuity in care.

Qualifications

• Current licensure in Florida (or eligibility) as a Family Nurse Practitioner.
• National certification from a recognized body such as AANP or ANCC.
• A strong commitment to patient education, health equity, and preventive practices.
• Ability to work independently as well as within a team-based care model, demonstrating excellent collaboration skills.

Preferred Experience

• A minimum of 2 years of experience in outpatient family medicine is highly preferred; however, recent graduates who share our passion for health and community may also be considered.
• Experience working with pediatric populations or school health settings is advantageous, as it aligns with our focus on family-centered care.
• Proficient in using EMR systems to manage patient information and track health outcomes effectively.

Compensation and Benefits
