Work Environment

This position is based onsite, with the majority of your work conducted through house calls to patients in the Greater Columbus area. Key Responsibilities

• Conduct thorough and comprehensive assessments of patients' acute and chronic health conditions during home visits, using your clinical expertise to evaluate symptoms.
• Develop, implement, and manage personalized treatment plans based on evidence-based practices, collaborating effectively with physicians and specialists.
• Order and interpret diagnostic tests, enabling swift and informed decision-making. Prescribe medications and therapies that align with patients’ health objectives.
• Educate patients and their families on managing their health conditions, emphasizing the importance of adherence to treatment plans and follow-up care.
• Document all patient interactions and care plans in the electronic medical record system accurately to maintain continuity of care and comply with regulatory standards.
• Engage in continuous quality improvement initiatives, contributing your insights to refine practices and enhance patient outcomes.
• Maintain up-to-date knowledge of healthcare trends, policies, and best practices, ensuring alignment with the latest medical guidelines.

Qualifications

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 220,000 based on your experience and qualifications.
