Work Environment

The role is set within a school facility, providing a unique environment for engaging with children, families, and educators. Our team works closely with school staff and community partners, maintaining a schedule that allows flexibility for direct care and community outreach. Regular hours are from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m. Participation in community health fairs and outreach events during weekends is encouraged to foster stronger health awareness.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse patient population, tailoring care to meet individual and family needs.
• Diagnose and manage acute and chronic conditions, delivering targeted preventive care and health promotion strategies.
• Work collaboratively with community organizations to design and implement health education programs that cater to the specific needs of the population.
• Advocate for culturally sensitive care and patients' rights, ensuring that every individual has access to the healthcare services they deserve.
• Coordinate referrals and follow-up care, facilitating seamless transitions and continuity for our patients.
• Maintain accurate and comprehensive medical records, adhering to all regulatory compliance requirements and best practices.
• Engage in community outreach efforts, participating in health fairs and outreach events, to extend our reach and impact within the community.
• Stay informed about community health trends, evidence-based practices, and pertinent policies to continually enhance service quality and effectiveness.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, with a particular focus on children and families within the school setting. Many of our patients come from underserved communities, which presents unique healthcare challenges and opportunities to make a meaningful difference in their lives. Your collaborative approach will ensure that patients receive comprehensive support tailored to their cultural and socio-economic backgrounds.

Qualifications

• A current certification as a Family Nurse Practitioner.
• An active nursing license in the state of New York.
• A Master's degree in Nursing from an accredited institution.
• A commitment to social justice and health equity with a proven track record of advocating for vulnerable populations.
• Exceptional interpersonal and communication skills, enabling effective interaction with a wide range of individuals and communities.

Preferred Experience

• At least 3 to 5 years of clinical experience, particularly in community health, public health, or school-based health settings.
• A background in program development, specifically in health education and outreach.
• Experience working with children and families, particularly in school or community healthcare settings.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications. In addition to a competitive salary, our benefits package includes:
