Work Environment

This role is primarily onsite, within our clinic in New York City, where you will engage directly with patients. You will have the opportunity to conduct house calls, providing care in patients' homes, which fosters a personal connection and allows for tailored healthcare delivery. The working hours are generally from 9:00 AM to 6:00 PM, with some evening clinics until 8:00 PM and occasional weekend community outreach events that help raise awareness of health issues and provide screenings to underserved populations.

Key Responsibilities

• Perform comprehensive health assessments and screenings for individuals and families, identifying specific health needs and concerns.
• Diagnose and manage both acute and chronic medical conditions through evidence-based practices, ensuring preventive care and promoting health wellness.
• Collaborate with multidisciplinary community organizations to develop and implement innovative health education programs tailored to the needs of our patient population.
• Advocate for the rights of patients, providing culturally competent care that respects diverse backgrounds and promotes health equity.
• Coordinate referrals to specialists and arrange for follow-up care to guarantee continuity and quality of care.

Qualifications

• Must hold an active certification as a Nurse Practitioner.
