In this full-time, onsite role, the NP will operate within an outpatient office setting, collaborating closely with a team of healthcare professionals to provide top-notch care to patients with complex cardiac conditions. The position offers a competitive salary range of USD 120,000 to 220,000 per year, based on experience and qualifications.

Key Responsibilities

• Conduct comprehensive patient assessments by obtaining thorough medical histories and performing detailed physical examinations.
• Order and interpret diagnostic tests, diagnosing and managing both chronic and acute cardiac conditions.
• Deliver patient care both collaboratively and autonomously, ensuring that treatment plans are created and adjusted based on individual patient needs.
• Engage in continuous communication with physicians, nurse practitioners, and registered nurses to refine patient treatment strategies and facilitate effective transitions of care.
• Educate patients and their families about disease processes, care management strategies, and preventive health measures, fostering a supportive environment for informed decision-making.
• Coordinate admissions to hospital services as necessary and collaborate with inpatient teams to ensure seamless patient discharges.
• Document all patient interactions using the electronic medical records (EMR) system, ensuring accuracy and compliance with documentation standards.
• Participate in ongoing clinical research activities, integrating evidence-based findings into everyday clinical practice to improve patient care.
• Maintain a safe and therapeutic environment that aligns with organizational values of patient-centered care.
• Take on additional responsibilities as assigned by the supervising physician or nurse leadership, ensuring flexibility and adaptability in a fast-paced healthcare environment.

Patient Population / Client Focus

As a Nurse Practitioner in our CACC, you will work primarily with patients suffering from a variety of cardiac conditions, including but not limited to heart failure, pulmonary arterial hypertension, and those who have undergone heart transplants. You will also be involved with patients undergoing advanced therapies, providing specialized care and support as they navigate complex treatment regimens.

Qualifications

• A Master’s degree in Nursing with training as a Nurse Practitioner focused on adult care.
• A minimum of two years of experience in a clinical setting, especially in cardiology or a related field.
• Active New York State Registered Professional Nurse and Nurse Practitioner licenses.
• NP Board certification and current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in utilizing electronic medical records systems along with familiarity with software such as MS Word, PowerPoint, and Excel.

Preferred Experience

Experience working with advanced Heart Failure patients, those with Pulmonary Arterial Hypertension, and managing care for individuals post-heart transplant is highly desirable. Familiarity with outpatient intravenous or inhaled medication support and mechanical circulatory support devices will be advantageous. Candidates who demonstrate a holistic understanding of the patient experience within the cardiology spectrum will be prioritized.
