Key Responsibilities

As a Nurse Practitioner, your day-to-day responsibilities will include but are not limited to:

• Conducting thorough assessments of patient health, including detailed histories and physical examinations.
• Developing individualized treatment plans, integrating patient feedback and preferences to ensure effective care.
• Monitoring and managing patients with chronic diseases, such as diabetes, hypertension, or heart disease, while coordinating services with other healthcare professionals.
• Providing education and resources to patients and families about health maintenance, disease prevention, and treatment options.
• Administering medications, as well as conducting diagnostic tests and interpreting results using established protocols.
• Documenting all patient interactions and care processes in our electronic health record system to ensure accurate and comprehensive patient records.

Patient Population / Client Focus

You will serve a diverse patient population, including adults and elderly individuals with a range of health conditions, ensuring culturally competent care. A key focus will be on addressing the unique healthcare challenges faced by vulnerable communities in Spring Valley. By proactively engaging with these populations, you will contribute directly to improving healthcare access and outcomes.

Qualifications

To be considered for this role, candidates must possess:

• A Master’s degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner.
• An active state nursing license in New York.
• Strong interpersonal and communication skills, enabling effective collaboration with patients and multidisciplinary teams.
• A commitment to health equity, with demonstrated sensitivity to the diverse needs of underserved populations.

Preferred Experience

Compensation and Benefits
