We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our team at Cherish Center Wellness in New Bedford, Massachusetts. Work Environment

This position is situated in our outpatient office, where you will work closely with a diverse team of healthcare professionals in a supportive and collaborative atmosphere. Our typical work schedule will see you from 9:00 a.m. to 6:00 p.m., with the occasional late evening clinic until 8:00 p.m., as well as participation in community outreach activities and health fairs during weekends. You can expect to engage in approximately 40 hours of work per week.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages, including families and individuals.
• Diagnose and treat both acute and chronic medical conditions, emphasizing preventive care and health promotion.
• Collaborate with community organizations to design and execute health education programs aimed at addressing specific community health needs.
• Advocate for patient rights and work to ensure access to healthcare services for our clients.
• Coordinate patient referrals and establish follow-up care plans to maintain continuity of care.
• Document patient information accurately, maintaining medical records in compliance with HIPAA regulations and professional standards.
• Engage in community outreach initiatives and health fairs, providing health screenings and education to raise awareness about health issues.
• Stay abreast of community health trends and practices, utilizing evidence-based approaches to improve patient outcomes.

Patient Population / Client Focus

Your work will primarily focus on serving vulnerable populations in New Bedford, where you will treat a range of health conditions while also addressing social determinants of health. You will work with diverse groups, including families facing economic and social challenges, providing culturally sensitive care that respects individual backgrounds and needs.

Qualifications

• Current certification as a Nurse Practitioner is required.
• Must possess an active Massachusetts nursing license.
• A Master's degree in nursing from an accredited institution is essential for this role.
• At least 4 years of clinical experience, ideally in community health or public health settings.
• Exceptional communication and interpersonal skills, with the ability to interact effectively with a diverse range of patients and colleagues.
• Strong commitment to social justice and health equity, driven to advocate for marginalized communities.

Preferred Experience

• Experience in preventive care and health education, particularly in community outreach initiatives.
• Familiarity with electronic medical records (EMR) systems to ensure effective documentation and patient management.
• Proven ability to work in a team-oriented environment, supporting a collaborative and interprofessional approach to patient care.

Compensation and Benefits

We offer a competitive salary range from USD 120,000 to 220,000 per year, based on your experience and qualifications.
