Work Environment

The Nurse Practitioner will work onsite at our Assisted Living Facility, which offers a supportive and nurturing environment for patients requiring assistance with daily activities and healthcare needs. Key Responsibilities

• Provide primary medical care to residents, addressing their physical and mental health needs.
• Conduct thorough initial examinations and ongoing assessments, utilizing evidence-based practices to evaluate patient conditions.
• Develop, implement, and review individualized treatment plans in collaboration with patients and their families, ensuring alignment with their health goals.
• Order, perform, and interpret diagnostic tests and radiographic studies to accurately identify and manage health issues.
• Monitor and operate medical equipment, ensuring functionality and safety in patient monitoring.
• Educate patients and caregivers about health conditions, treatment options, and lifestyle modifications to empower them in managing their health.
• Maintain accurate and up-to-date patient medical histories and documentation in accordance with regulatory standards.
• Make timely referrals to specialists when necessary, coordinating care transitions effectively to avoid any disruption in treatment.

Patient Population / Client Focus

This role primarily focuses on elderly patients and individuals with chronic health conditions residing in the assisted living facility. The Nurse Practitioner will foster meaningful relationships with residents and their families, understanding their unique medical histories and personal care preferences, thus enhancing overall satisfaction and care outcomes.

Qualifications

• Active Nurse Practitioner certification and a state nursing license are mandatory.
• A Master’s Degree in Nursing from an accredited institution is required.
• Empathy and strong interpersonal skills to establish rapport with patients and their families.

Preferred Experience

• A minimum of 3-5 years of clinical experience, preferably within community health, public health, or geriatric settings, is highly desired.
• Experience with electronic health records (EHR) and familiarity with practice management software will be an advantage.
• Previous experience in an assisted living or similar environment is a plus.

Compensation and Benefits
