Key Responsibilities

• Deliver comprehensive and individualized patient care, ensuring ongoing assessments and establishing partnerships with families to promote continuity and quality of care.
• Implement cost-effective primary care services, emphasizing preventative measures and chronic disease management to improve overall health outcomes.
• Make appropriate referrals to specialists as needed, ensuring follow-up and coordination of care to support patient journeys.
• Maintain meticulous electronic medical records in accordance with organizational standards, ensuring the highest levels of accuracy and compliance.
• Facilitate engagement between patients, families, and multidisciplinary care teams, fostering a collaborative approach to ongoing health and wellness.
• Engage with the interdisciplinary teams and resources available through Flowing Help, contributing to a holistic care model.
• Participate in training programs designed to enhance skills in community health, care for underserved populations—including homeless individuals, refugees, and veterans—and advocate for equity within health care services.
• Lead outreach initiatives targeting vulnerable populations, focusing on increasing access to care for those experiencing homelessness.
• Provide exceptional primary care to patients across all age ranges, from infants to older adults, with sensitivity to their unique physical and emotional needs.
• Take on leadership roles within cross-functional patient care teams, promoting best practices and fostering a patient-centric approach to health care.

Patient Population / Client Focus

The Nurse Practitioner will cater to a diverse population, primarily comprising school-aged children and their families. This includes addressing acute and chronic health issues, conducting routine health assessments, and facilitating health education. The role extends to engaging with adolescents and their unique health needs, along with providing comprehensive care for both pediatric and adult patients, establishing a continuum of care across different life stages.

Qualifications

• A valid Nurse Practitioner license in California is required.
• A Bachelor’s Degree in nursing or a related field is mandatory, with advanced clinical training.

Compensation and Benefits

• The annual salary range for this role is USD 120,000 – 130,000.
