This position is based onsite at a local school in Oakland, where you will work collaboratively with a team of healthcare professionals to deliver quality medical care. Key Responsibilities

• Conduct comprehensive assessments and diagnoses of both acute and chronic medical conditions, utilizing evidence-based practices to inform clinical decisions.
• Develop individualized treatment plans, order necessary diagnostic tests, and interpret results to guide patient care.
• Prescribe medications and advise on additional therapies to promote health and recovery, ensuring adherence to medication protocols and regulations.
• Educate students and their families on disease prevention, effective health promotion strategies, and healthy lifestyle choices, empowering them to take charge of their own health.
• Coordinate care with other healthcare professionals, including school nurses, counselors, and community resources, to provide a seamless continuum of care for students.
• Maintain accurate and detailed medical records, ensuring compliance with state regulations and organizational policies.
• Engage in ongoing quality improvement initiatives aimed at enhancing service delivery and patient satisfaction.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on the health care needs of school-aged children, addressing a variety of health concerns ranging from routine wellness checks to chronic disease management. This role includes preventive screenings, immunizations, and managing conditions such as asthma, diabetes, and attention disorders, ensuring that students can participate fully in their educational endeavors.

Qualifications

• A valid Nurse Practitioner certification and an active California nursing license are required.
• A Master’s degree in nursing from an accredited program is mandatory.

Compensation and Benefits
