Key Responsibilities

• Utilize relationship-based medicine to assess and treat patients, focusing on holistic approaches that address chronic conditions and preventative care.
• Engage with patients through both individual and group sessions, developing care plans that align with their unique health goals.
• Provide education on health management, encouraging patients to take an active role in their own care through self-management strategies and community resource utilization.
• Document all patient encounters and maintain accurate records in accordance with industry standards, ensuring that documentation is completed within 72 hours.
• Prescribe medications and develop treatment plans that minimize risks while addressing health issues, adhering to state licensure and organizational protocols.
• Respond to emergencies with confidence, ensuring patient safety and well-being even in high-stress situations, and recognizing when further consultation is required.
• Order necessary therapies, such as occupational and physical therapy, coordinating comprehensive treatment processes tailored to individual needs.

Patient Population / Client Focus

You will be working with a diverse patient population, including children, adolescents, and adults facing a range of medical complexities. Our patients often present with chronic illnesses that require both compassionate care and proactive management. The emotional challenges faced by many in this demographic necessitate a healthcare provider who can deliver support with empathy and understanding, while also maintaining a solid patient-provider dynamic.

Qualifications

• A Bachelor’s Degree in Nursing or a related field.
• Active NP license and current certification from either the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP).
• Basic Life Support (BLS) and CPR certifications
• Independent family practice experience is preferred, with a focus on wellness care, chronic care management, and urgent care situations.
• Ability to handle complex patient interactions and effectively communicate with individuals of various ages.

Preferred Experience

• Experience as an independent practice provider, where applicable based on state regulations.
• Federal Drug Enforcement Administration (DEA) number is preferred to facilitate prescribing as needed.
