Position Overview
We are seeking a compassionate and highly skilled Wound Care Nurse Practitioner/PA to join our mobile care team. You will provide advanced wound care to patients across Southern California, helping manage and treat chronic and complex wounds directly in the home or care facility setting.
Key Responsibilities
Conduct thorough patient assessments and develop individualized wound care plans
Perform advanced wound treatments including debridement, dressing changes, and infection management
Collaborate with physicians and healthcare teams to ensure continuity of care
Educate patients and caregivers on wound prevention and proper care techniques
Accurately document visits and treatment progress using EMR systems
Stay up to date on best practices and new developments in wound care
Qualifications
Licensed Nurse Practitioner in California
Wound care experience preferred (training available for the right candidate)
Familiarity with DME and chronic care is a plus
Strong communication and organizational skills
Ability to work independently and manage your own schedule
Must have a valid driver's license and reliable transportation
