Key Responsibilities
- Assess patients' level of functioning upon physician orders and develop individualized treatment plans based on evaluation results and patient/family goals.
- Plan, implement, and document occupational therapy treatments in accordance with applicable federal, state, and local standards, regulations, and guidelines.
- Participate in the interdisciplinary team process, including team conferences, discharge planning, patient and family education, and adaptive equipment recommendations.
- Supervise Certified Occupational Therapy Assistants (COTAs) and OT/COTA students in the application of occupational therapy theory and techniques.
- Complete all required documentation including initial evaluations, daily progress notes, treatment programs, and discharge summaries according to department policy and within defined timeframes.
- Promote patient and employee safety through adherence to safe patient handling, infection control, and workplace hazard reporting practices.
Required Qualifications
- Current licensure or eligibility for licensure as an Occupational Therapist in the state of Wisconsin.
- Demonstrated ability to perform comprehensive patient evaluations and develop appropriate, evidence-based treatment plans.
- Experience or ability to supervise COTAs, OT students, and COTA students in a clinical setting.
- Strong documentation skills with the ability to complete progress notes, discharge summaries, and treatment records accurately and on time.
- Ability to communicate effectively with patients, families, and interdisciplinary team members at varying levels of understanding.
Preferred Experience / Skills
- Prior experience in a rehabilitation or post-acute care setting.
- Familiarity with interdisciplinary team collaboration and structured discharge planning processes.
- Experience with adaptive equipment assessment, issuance, and patient/caregiver training.
