Key Responsibilities

• Dispense prescription medications to patients accurately, ensuring proper dosage, safety, and adherence to therapeutic regimens.
• Conduct thorough medication reviews and consult with patients and healthcare providers to educate them about drug therapies, potential side effects, and drug interactions.
• Verify and validate prescription orders for completeness, compliance with legal standards, and appropriateness based on patient-specific factors.
• Engage with medical staff in multidisciplinary rounds to provide insights regarding medication management and to make evidence-based recommendations.
• Oversee the management of pharmacy inventory, ensuring timely procurement of medications and maintaining adequate stock levels.
• Adhere to and enforce federal, state, and local regulations regarding pharmacy practice and medication distribution.

Patient Population / Client Focus

As a Clinical Pharmacist, you will be responsible for tailoring pharmacotherapy regimens to meet the unique needs of patients, including the elderly, pediatrics, and individuals with multiple comorbidities. Qualifications

• A Doctor of Pharmacy (PharmD) degree from an accredited college or university is required.
• Must possess an active pharmacy license in New York State.

• Exceptional communication skills, enabling effective interaction with patients, families, and healthcare providers.

Compensation and Benefits
