Job Description
Physical Therapists Needed in Port Jervis, New York- Full-time opportunity
We are seeking a skilled Physical Therapist with experience in a skilled nursing facility. The ideal candidate will help rehabilitate individuals dealing with physical impairments so they can return to their normal activities.
Job Type: Full Time
Schedule: Monday – Friday 9:00 AM-5:00 PM
Compensation: $100,000 – $120,000 per year + benefits
Patient Volume: 2 patients per hour
Job Duties:
- Review patients' medical history, referrals, and notes from doctors and other healthcare professionals
- Assess patients' abilities, limitations, and needs by observing their movements and listening to their concerns
- Develop individualized plans of care that outline patient goals and expected outcomes based on their assessments
- Work closely with physicians, nurses, and other therapists to ensure comprehensive care
- Guide residents through exercises, stretches, and other therapeutic interventions to improve strength, flexibility, and coordination
- Assist residents with the proper use of equipment such as wheelchairs, walkers, canes, and grab bars.
- Provide massage, manual therapy, or other hands-on techniques to ease pain and promote healing
- Educate residents and their families about the recovery process, fall prevention, pain management, and strategies for improving mobility
- Evaluate and record patients' progress, modifying the plan of care as needed
- Document their findings and communicate them to the healthcare team and families
- Identify potential hazards in the home and recommend safety equipment and home modifications as needed
- Keep track of goals and progress while adapting treatment plans accordingly
Requirements:
- Valid Physical Therapy license in the state of New York
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited institution
- Clinical experience in a skilled nursing facility (preferred but not required)
- Strong communication and interpersonal skills
- Ability to develop and implement patient-specific treatment plans
- Passion for helping patients achieve meaningful recovery
- Ability to work independently and as part of a collaborative team
