PCM is hiring full-time and part-time Physical Therapists in/near Los Alamos, NM to provide in-home healthcare services to help those who have served our country!
We provide in-home care to former Nuclear Weapons Workers who are suffering from chronic and terminal illnesses, as a result of their previous work environment.
Our Physical Therapists conduct in-home visits for our clients. The PT visits are conducted weekly and typically last 1-2 hours per visit.
Qualifications
- Graduate of a physical therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association
- Current, unrestricted license as a physical therapist/physical therapist assistant in the state(s) of practice
- Minimum of two (2) year's experience in health care field
- Home care experience preferred
- Current CPR certification
- Strong communication and interpersonal skills
- Demonstrates ability to work independently with minimal supervision
- Licensed driver with good driving record and automobile insured in accordance with state and/or Agency requirements
Essential Functions/Areas of Accountability
- Independently and responsibly perform assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving physical therapy services under a medically approved plan of care:
- Complete initial assessment within forty-eight (48) hours of referral unless other arrangements are made
- Develop a therapy plan to provide direction to other health team members providing care
- Review and update therapy plan per Agency policy and client need
- Document all findings, plans, interventions, and outcomes and submit documentation to the Agency within seven (7) days
- Communicate plans and changes to the physician and to the client case manager and other caregivers through the care plan, progress notes, and participation in care conferences
- Communicate changes in schedule and physician orders to Agency on the day the changes are made
- Assess and evaluate status of assigned clients on an ongoing basis:
- Collect objective and subjective client information
- Analyze, report and record data
- Validate, refine and modify data
- Utilize all data to identify and document current therapy needs of assigned clients
- Collaborate with client, family/others, and other members of the healthcare team to identify client needs, and establish realistic goals of care
- Review plan on a regular basis to evaluate client response to plan, prioritize needs, and modify approaches to meet client’s current needs
- Communicate interventions and client response to interventions effectively and in accordance with Agency requirements:
- Report significant changes and needs to physician and in accordance with Agency requirements
- Maintain client records and clinical notes, showing systematic assessment, planning, intervention, and evaluation
- Coordinate PT services for the client as needed
- Participate in team meetings and clarify individualized reporting responsibilities with all new assignments
- Share areas of expertise among and between teams
- Initiate interdisciplinary referrals
- Participate in discharge planning and document in client record
- Document legibly and according to Agency documentation guidelines and standards
- Initiate teaching appropriate to the client’s needs and within the context of his or her environment and learning ability
- Assess knowledge level of client and identify learning needs
- Plan, teach, supervise, and counsel client about physical therapy needs to promote improvement or recovery, symptom management of disease process, and community resources
- Evaluate the effectiveness of the teaching plan and establish approaches for others to follow in teaching the client
- Assist in the development, implementation, and evaluation of the ongoing Agency performance improvement program
- Participate in quarterly medical record review
- Participate in other PI activities as requested
- Participate with interdisciplinary team to ensure consistency in data collection methods
- Promote personal safety and a safe environment for clients and coworkers:
- Demonstrate knowledge of safety/infection control practices by complying with Agency policies and procedures
- Recognize and respond to potentially unsafe situations
- Demonstrate safe practice in the use of equipment
- Assess safety of environment and take initiative to help prevent accidents and promote safety
- Supervise physical therapy assistants:
- Instruct and supervise PT assistants in certain phases of the physical therapy services they may provide to clients
- Perform in-home supervision while PT assistant is rendering care every four (4) to six (6) visits, minimally once every two (2) months – supervision does not constitute treatment
- Evaluate performance and provide timely reviews as delegated
- Available by phone at all times when a PT assistant is providing treatment for a client
- Complete an on-site functional assessment, review, and revision of the treatment plan and an assessment of utilization of community resources
- Perform job in compliance with Agency policies and procedures, professional standards of practice and state-specific regulatory requirements
- Attend meetings and required continuing education offerings
- Accept responsibility for personal and professional development and identify learning/ developmental needs to supervisor
- Perform other duties and responsibilities as deemed necessary
