Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for clinical care.
- Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Illinois.
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines.
- Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards.
- Demonstrated commitment to patient safety and safe patient handling practices.
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Active involvement in continuing education and professional development activities.
