Work Environment

The physical therapist role operates primarily onsite within our home care facility, delivering support directly in patient homes. Key Responsibilities

• Conduct comprehensive patient assessments to identify functional limitations and develop tailored treatment plans.
• Implement effective therapy interventions, monitoring patient progress and adjusting plans as necessary to achieve optimal outcomes.
• Provide expert consultation to healthcare providers to ensure a seamless continuum of care for patients.
• Document all evaluations, treatment plans, progress notes, and discharge summaries in accordance with applicable laws and professional standards.
• Engage with patients and their families to educate them on treatment strategies, home exercises, and self-care techniques.
• Participate in community outreach initiatives to promote health awareness and physical therapy benefits.
• Supervise and guide support staff, enhancing their skills and knowledge to improve service quality.
• Maintain open communication with an interdisciplinary team, including physicians and referral sources, ensuring cohesive patient management.
• Stay updated on industry trends and contribute to the development of clinical practices and departmental objectives.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.

Compensation and Benefits

The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
