Work Environment

This position is entirely onsite, requiring you to travel to patients' homes in and around Reading, PA. The home care setting allows for personalized, one-on-one therapy sessions that can significantly impact patient progress and satisfaction. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical abilities, mobility limits, and rehabilitation needs.
• Develop individualized treatment plans based on initial assessments, with benchmarks for progress tailored to each individual's condition and recovery goals.
• Implement hands-on treatment techniques and therapeutic exercises to improve strength, flexibility, and balance, and alleviate pain.
• Provide education on proper body mechanics, injury prevention, and home exercises to promote long-term recovery and independence.
• Continuously monitor and document patients' progress while adjusting treatment plans in response to their evolving needs.
• Collaborate closely with other healthcare professionals, including physicians and occupational therapists, to ensure a holistic approach to patient care.
• Maintain detailed and accurate patient records in compliance with healthcare regulations and standards.

Patient Population / Client Focus

Your caseload will primarily include patients recovering from surgery, those with chronic pain conditions, and individuals with mobility challenges due to injuries, illness, or age-related issues. By focusing on their unique challenges and goals, you will play a vital role in restoring their quality of life and ensuring they can engage in daily activities with confidence. Our diverse patient population calls for therapists who are adaptable, empathetic, and skilled in communicating effectively with individuals of various backgrounds and needs.

Qualifications

• Bachelor's or Master’s degree in Physical Therapy from an accredited institution.
• Active state licensure as a Physical Therapist in Pennsylvania.
• Board certification from the American Physical Therapy Association (APTA).
• A minimum of one year of experience in a clinical setting, preferably in home care or outpatient rehabilitation.
• Proficiency in a variety of physical therapy modalities and treatment techniques.
• Strong analytical, critical thinking, and problem-solving skills to create effective rehabilitation plans.
• Excellent communication and interpersonal skills to foster positive relationships with patients and their families.

Preferred Experience

While a minimum of one year of clinical experience is required, candidates with experience in home care rehabilitation or a specialized focus on post-surgical recovery, conservative management of chronic conditions, or pediatric therapies will be particularly valued. Familiarity with electronic health record (EHR) systems and telehealth platforms is also beneficial as we continue to integrate technology into our practice.

Compensation and Benefits
