This position is entirely onsite, requiring you to travel to patients' homes in and around Reading, PA. Your caseload will primarily include patients recovering from surgery, those with chronic pain conditions, and individuals with mobility challenges due to injuries, illness, or age-related issues. By focusing on their unique challenges and goals, you will play a vital role in restoring their quality of life and ensuring they can engage in daily activities with confidence. While a minimum of one year of clinical experience is required, candidates with experience in home care rehabilitation or a specialized focus on post-surgical recovery, conservative management of chronic conditions, or pediatric therapies will be particularly valued.
