Work Environment

The successful candidate will operate primarily in a home care setting, delivering targeted physical therapy interventions directly in patients' homes. Key Responsibilities

• Conduct thorough physical evaluations of patients to assess their functional abilities, mobility limitations, and therapeutic needs.
• Develop and implement individualized treatment plans based on assessment findings, encompassing various techniques such as therapeutic exercises, manual therapy, and modalities like ultrasound or electrical stimulation.
• Educate patients on strategies to manage their conditions, improve their physical function, and prevent future injuries, emphasizing proper body mechanics and ergonomic practices.
• Monitor and document patients' progress meticulously, adjusting treatment plans as necessary to ensure optimal rehabilitation outcomes.
• Coordinate care with other healthcare providers, ensuring a multidisciplinary approach to patient care that encompasses referrals and integrated support.
• Maintain comprehensive records of all treatments and interactions, ensuring compliance with best practices and regulatory requirements.

Patient Population / Client Focus

Our physical therapists cater to a diverse range of patients, including those recovering from surgery, individuals dealing with chronic pain conditions such as osteoarthritis, and clients managing acute injuries such as fractures or sprains. We also work with elderly patients to address balance issues and gait deficits, focusing on strategies that enhance their quality of life and independence. Qualifications

• Bachelor's or Master’s degree in Physical Therapy from an accredited institution.
• Current valid state licensure to practice as a Physical Therapist.
• Board certification by the American Physical Therapy Association (APTA).
• At least one year of relevant clinical experience in a physical therapy setting, with a strong emphasis on home care methodologies.
