Work Environment

This role is strictly onsite, focusing on delivering services in patients’ homes throughout The Villages area. Our physical therapists work closely with home health aides and other healthcare professionals to ensure integrated and cohesive care. Key Responsibilities

As a Home Health Physical Therapist, you will:

• Direct physical therapy treatments aimed at preventing disabilities, relieving pain, and restoring muscle function in accordance with each patient’s care plan.
• Supervise physical therapy assistants to ensure accurate implementation of treatment protocols.
• Conduct thorough evaluations of clients' function levels through diagnostic tests to inform individualized treatment plans.
• Analyze patient home environments and provide recommendations to promote independence and safety during rehabilitation.
• Administer therapeutic exercises, manual therapy, and other physical agents as outlined in physician orders.
• Evaluate, fit, and adjust orthotic devices and recommend modifications to improve patient mobility.
• Document and maintain clinical records, including evaluations, care plans, and progress notes, with strong attention to detail.
• Participate in interdisciplinary team meetings to discuss patient progress and care strategies, ensuring alignment across treatment modalities.
• Communicate effectively with patients and families regarding treatment goals, home exercise programs, and the use of therapy devices.
• Complete administrative tasks, such as timely documentation and management of clinical and payroll orders.

Patient Population / Client Focus

Our patient population includes individuals of varying ages and physical conditions requiring personalized rehabilitation services. You will work with patients recovering from surgeries, managing chronic conditions, or dealing with neuromuscular disorders. By focusing on individual capabilities and lifestyle goals, you will play a crucial role in their return to maximum functionality and independence.

Qualifications

To be considered for this role, candidates must possess:

• A Doctorate degree in Physical Therapy (DPT) and a valid PT license in the State of Florida.
• Current CPR certification and a valid driver’s license with automobile insurance.
• Strong interpersonal skills with a focus on patient engagement and communication.
• The ability to adapt to various home environments and navigate challenges inherent in home care settings.

Preferred Experience

While the minimum requirement is two years of related physical therapy experience, candidates with a solid background in acute care or home care settings will be favored. Experience in managing diverse populations with a variety of health conditions is highly beneficial.
