Carle Physical Therapy is seeking a dedicated Full-Time Home Care Physical Therapist to join our team in Fairview, New Jersey. Work Environment

This full-time position is entirely onsite, allowing therapists to provide care directly in patients' homes. The work environment is dynamic, requiring flexibility and excellent interpersonal skills as therapists engage with patients from diverse backgrounds. Weekly work hours total 40 hours, with variable weekend availability depending on patient scheduling needs. Key Responsibilities

• Conduct comprehensive examinations and evaluations to assess patients' functional limitations and disabilities.
• Develop and implement personalized treatment plans that align with manufacturers’ guidelines and comply with all applicable laws and regulations.
• Provide ongoing consultation and collaborative care planning with healthcare providers to ensure a seamless continuum of care for each patient.
• Deliver patient education and engage with families and caregivers regarding therapy goals, treatment protocols, and discharge plans.
• Maintain accurate documentation of assessments, treatment plans, outcomes, and charges, adhering to organizational and professional standards.
• Participate in staff meetings and contribute to continuous quality improvement initiatives and marketing efforts.
• Supervise and guide assigned support staff, ensuring efficient management of departmental resources.
• Engage in self-directed learning and pursue continuing education opportunities to enhance professional expertise.

Patient Population / Client Focus

As a Physical Therapist at Carle Physical Therapy, you will work primarily with patients recovering from orthopedic conditions, post-surgical rehabilitation, and injuries that impact their mobility and quality of life. Your understanding of diverse patient needs and cultural competencies will ensure that you effectively tailor treatment plans to suit each individual's circumstances.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited institution.
• Active state license in Physical Therapy required for practice.
• Certification in Basic Life Support (BLS) is preferred.
• Demonstrated effective communication skills, both written and verbal, are essential for collaborating with patients, families, and interdisciplinary teams.
• Proficiency with electronic medical records (EMR) systems and basic word processing software is required.

Preferred Experience

A minimum of two years of professional experience in physical therapy, preferably with a focus on home health or rehabilitation services. Prior experience in multidisciplinary team settings and familiarity with community health resources would be an asset to effectively support patient well-being.

Compensation and Benefits

The compensation for this full-time position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
