Key Responsibilities

• Develop and implement individualized treatment plans for clients aimed at preventing disabilities, alleviating pain, and improving muscle function.
• Assess and monitor patient progress using diagnostic tests and other evaluation methods, adjusting treatment plans as necessary.
• Oversee Physical Therapy Assistants in the execution of their responsibilities, ensuring compliance with established treatment protocols.
• Conduct home evaluations to provide insights on environmental adaptations that bolster patient independence.
• Administer therapeutic exercises, traction, massage, and mechanical treatments, while instructing patients and families on effective techniques.
• Engage in interdisciplinary team meetings to discuss case reviews and collaborate on comprehensive treatment strategies.
• Document patient progress and treatment outcomes through clinical records, including initial evaluations, OASIS assessments, and discharge summaries.
• Provide clear guidance to clients and their families regarding the use and care of physical therapy devices, enhancing their understanding of the rehabilitation process.
• Perform regular functional assessments and ensure that care plans reflect the ongoing needs and goals of the client.
• Complete clinical and administrative documentation in a timely manner, adhering to organizational policies.

Patient Population / Client Focus

In this role, you will primarily work with a diverse outpatient population ranging from post-operative recovery clients to those managing chronic conditions. Each patient undergoes a thorough assessment to tailor treatment options that best fit their individual lifestyles. Your expertise will be critical in guiding patients through rehabilitation processes that empower them to regain independence and enhance their quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapy licensure in the state of Texas, in good standing.
• Minimum of one year of clinical experience, preferably in an acute care or home care setting.
• Current CPR certification is required.
• Possession of a valid driver’s license and automobile insurance for the purpose of traveling to patients’ homes.
• Strong interpersonal skills with an emphasis on patient and family engagement.
