Work Environment

This position is onsite, requiring you to visit patients in their homes throughout Chesapeake and surrounding areas. You will work within a supportive environment comprised of fellow health professionals who prioritize collaboration and patient-centered care. Your scheduling is flexible, allowing you to choose hours that best fit your lifestyle from Monday to Friday.

Key Responsibilities

• Conduct initial evaluations for each patient to assess their physical condition and therapeutic needs.
• Design and implement tailored treatment plans that incorporate various therapeutic methods such as exercises, manual therapy, and assistive equipment.
• Monitor patient progress meticulously and adjust treatment strategies based on their responses and evolving needs.
• Educate patients, families, and caregivers about therapy techniques, assistive devices, and home exercises to ensure continuity of care.
• Document each session comprehensively, tracking patient progress toward therapeutic goals as required by state and federal regulations.
• Maintain your professional skills by attending workshops, and participating in professional societies relevant to physical therapy.

Patient Population / Client Focus

You will be engaged with a diverse range of patients, including children and adults recovering from surgery, injury, or illness. Our multi-faceted approach allows for customized rehabilitation plans tailored to each individual’s diagnosis and lifestyle, ensuring effective recovery and improved function.

Qualifications

• A valid Bachelor's Degree in Physical Therapy is required.
• Current state license to practice physical therapy.
• Demonstrated proficiency in verbal and written communication, crucial for effective patient interaction and documentation.
• Computer literacy, including proficiency with word processing and documentation software.
• Ability to be flexible and agile in adapting treatment approaches to meet patient needs.

Preferred Experience

While prior work experience in physical therapy is not mandatory, candidates who have completed internships or clinical rotations in home care settings will be prioritized. Compensation and Benefits
