Work Environment

This position requires you to work onsite, primarily in patients' homes, where you will have the opportunity to implement therapy in customized environments that cater directly to patient needs. Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations, assessing their needs based on a variety of diagnoses.
• Develop and implement individualized treatment plans tailored to each patient's therapeutic goals, utilizing methods such as exercises, hands-on interventions, and modern rehabilitation equipment.
• Monitor and document treatment progress, ensuring that all patient responses and adjustments to therapy plans are accurately recorded.
• Provide education and training for patients and caregivers about effective exercises, recovery techniques, and the use of assistive devices.
• Regularly re-evaluate patient progress and modify treatment plans as necessary to optimize patient outcomes.
• Participate actively in continued education and professional development to enhance skills and remain informed about the latest advancements in physical therapy.

Patient Population / Client Focus

Our skilled physical therapists cater to a varied patient demographic, including children and adults with unique rehabilitation needs stemming from injuries, surgeries, or chronic health issues. Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current and valid state license to practice as a physical therapist.

• Proficient in using electronic health record (EHR) systems for documenting patient progress and treatment plans.

Preferred Experience

Compensation and Benefits
