The selected candidate will work onsite in our outpatient office, which provides a dynamic and engaging atmosphere for both staff and patients. Physical therapists at Trails Physical Therapy work closely with each other, administrative staff, and healthcare providers to ensure a comprehensive and holistic approach to patient care.
Deliver compassionate and effective patient care in alignment with established treatment plans for both acute and outpatient cases.
Conduct thorough evaluations that include reviewing patient histories, performing assessments, and determining appropriate treatment recommendations.
Implement and adjust individualized treatment programs based on ongoing assessments and feedback from patients.
Document all interactions and care activities diligently, ensuring compliance with regulatory standards while maintaining clarity in the patient's continuity of care.
Take an active role in discharge planning and making follow-up care recommendations for ongoing support and patient wellness.
Maintain compliance with corporate and clinical training requirements as well as local, state, and federal regulations.
Foster open communication with co-workers, physicians, patients, and their families to collaborate effectively on care strategies and treatment plans.
Participate in team meetings and training sessions to remain informed on organizational initiatives and best practices.

Must be a graduate of an accredited Physical Therapy program.
Doctoral degree in Physical Therapy (DPT) is mandatory.
Possession of a valid physical therapy license in accordance with New Jersey state regulations.
Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
A minimum of 2 years of clinical experience as a licensed Physical Therapist.
Experience in an outpatient setting is highly desirable.
Specializations in orthopedic rehabilitation might be an advantage, reflecting a strong understanding of patient-centric therapy approaches.
