Work Environment

Your day-to-day activities will take place onsite at our state-of-the-art Rehabilitation Center located in Decatur, Illinois. Key Responsibilities

• Conduct in-depth evaluations of patients by reviewing their medical history, performing comprehensive assessments, and developing individualized treatment plans based on assessment findings.
• Communicate evaluation outcomes and treatment plans clearly to patients and their families, involving them in the goal-setting process.
• Implement effective treatment protocols tailored to patient needs, adapting strategies based on changes in patient condition or the evolving healthcare landscape.
• Collaborate with other healthcare providers to ensure a coordinated approach to patient care, documenting any changes in condition requiring modifications to treatment plans.
• Manage discharge planning and follow-up care, ensuring all documentation accurately reflects patient progress and care activities.
• Maintain effective time management, ensuring productivity expectations are met while keeping an accurate schedule.
• Foster a positive patient experience and ensure accessibility to care through flexibility and adaptability in your approach.
• Engage actively in departmental goals, contributing to both individual and collective success within the rehabilitation center.
• Participate in ongoing professional development and serve as a clinical instructor for students pursuing their education in physical therapy.

Patient Population / Client Focus

You will be working with a wide array of patients suffering from various physical injuries and disabilities, including post-surgical rehabilitation, sports injuries, and chronic pain conditions. Our center strives to provide the highest quality of care, focusing on evidence-based practices that facilitate optimal recovery and enhance the well-being of our clients.

Qualifications

• Bachelor's degree in Physical Therapy. A Doctorate in Physical Therapy is preferred.
• Current state license to practice as a physical therapist in Illinois.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
• Prior experience in a physical therapy setting is not mandatory, making this an excellent opportunity for recent graduates.

Preferred Experience

• Experience in outpatient rehabilitation is a plus but not required.
• Familiarity with electronic health records (EHR) systems and documentation standards is beneficial.

Compensation and Benefits
