Work Environment

This position is onsite and involves providing care directly within the homes of our patients. Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environment, assessing their functional capabilities and needs for recovery.

• Monitor and document patient progress meticulously, making adjustments to treatment plans as necessary to achieve optimal outcomes.

• Collaborate with interdisciplinary healthcare teams and maintain effective communication to ensure patient-centered care.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is required.
• Current state license to practice Physical Therapy in Florida.

Compensation and Benefits
