Work Environment

This full-time position is conducted onsite, directly in patients’ homes throughout the Front Royal area. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical history, performing physical assessments, and establishing treatment plans to address individual patient needs.
• Communicate evaluation findings and treatment plans clearly to both patients and their families, ensuring that they are involved in setting realistic rehabilitation goals.
• Implement evidence-based treatment protocols tailored to each patient’s unique presentation and adapt as necessary based on patient progress and feedback.
• Collaborate with other healthcare professionals to provide comprehensive care, recognizing when modifications to the treatment plan or additional consultations are necessary due to changes in the patient’s condition.
• Ensure meticulous documentation of patient care, including evaluations, treatment sessions, progress notes, and discharge planning, adhering to healthcare regulations and standards.
• Manage your schedule effectively to meet productivity expectations while providing high-quality patient care; this involves punctuality in meetings and involvement in community outreach initiatives.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited institution.
