Work Environment

This role offers a structured yet dynamic work setting. As a Physical Therapist providing at-home services, you will have a stable weekday schedule, working Monday through Friday from 9:00 AM to 5:00 PM. Your caseload will be manageable, allowing you to focus on delivering exceptional care without the risk of burnout. You will be supported by a competent team that promotes collaboration and clinical development.

Key Responsibilities

• Conduct thorough evaluations and develop individualized care plans for patients based on their specific needs and rehabilitation goals.
• Provide one-on-one treatment sessions, focusing on enhancing mobility and functionality while ensuring patient engagement throughout the recovery process.
• Monitor patient progress, adjusting treatment plans as necessary and documenting all interactions and interventions in our electronic medical record system.
• Educate patients and their caregivers on the rehabilitative process, exercises, and home care strategies to promote continued progress.
• Communicate effectively with interdisciplinary team members to ensure consistency and quality of care across all treatment settings.

Patient Population / Client Focus

Your caseload will consist of diverse patients coping with various physical conditions, including post-operative recovery, chronic pain management, and pediatric developmental delays. Our focus is on one-on-one care that fosters strong therapeutic relationships and helps patients regain their independence and improve their daily functioning.

Qualifications

• Minimum of a Bachelor’s Degree in Physical Therapy.
• Current and valid license to practice as a Physical Therapist in Colorado.
• Strong foundational knowledge of physical therapy principles and techniques.

Preferred Experience

• A minimum of 2 years of clinical experience in a physical therapy setting, preferably with home care exposure.
• Experience using electronic medical record systems; familiarity with WebPT is an advantage.
• Demonstrated ability to engage patients and reduce barriers to effective rehabilitation.

Compensation and Benefits

The compensation for this role ranges from $90,000 to $110,000 per year, commensurate with experience and qualifications.
