Full-Time Physical Therapist - Senior Care at an Assisted Living Facility

Position Overview

We are seeking a compassionate and skilled Physical Therapist to join our dedicated team at an assisted living facility in Gresham, Oregon. In this full-time role, you will play a crucial part in enhancing the quality of life for our senior residents through personalized physical therapy services that prioritize their mobility and overall well-being.

Work Environment

This position is based onsite in our outpatient office, providing a warm and welcoming atmosphere where residents trust us to deliver quality care. You will work Monday to Friday, from 9 AM to 5 PM, allowing for a balanced work-life structure. Our clinic is outfitted with modern therapeutic tools and an efficient electronic medical records (EMR) system, ensuring streamlined operations that enhance patient care.

Key Responsibilities

• Conduct detailed assessments of residents' physical abilities and limitations through comprehensive evaluations, including range of motion, strength, coordination, and functional mobility.
• Create and implement personalized treatment plans that align with the residents' goals, addressing their unique needs and promoting optimal rehabilitation outcomes.
• Facilitate effective transitions for residents returning home or moving back to assisted living from hospitals or skilled nursing facilities, providing support to ensure their comfort and confidence.
• Deliver therapeutic interventions focused on enhancing balance, gait training, pain management, and overall functional mobility, utilizing evidence-based practices.
• Collaborate closely with interdisciplinary teams, including nurses, physicians, and other therapists, to ensure continuity of care and achieve cohesive treatment goals.
• Educate residents, caregivers, and families on safe exercise techniques, transfer methods, and adjustable safety measures tailored to individual needs.
• Thoroughly document treatment progress and adjust care plans as needed, adhering to Medicare regulations and our facility’s protocols to ensure compliance and optimal service delivery.

Patient Population / Client Focus

You will primarily work with older adults residing in our assisted living facility. This demographic often faces mobility challenges, chronic pain, and post-acute recovery needs. By focusing on their individual strengths and limitations, you will help empower them to regain and maintain independence, which is fundamental for their quality of life.

Qualifications

• A Bachelor’s Degree in Physical Therapy from an accredited program (DPT, MPT, or BSPT).
• An active Physical Therapist license in Oregon or eligibility for licensure in the state.
• Strong interpersonal and communication skills, allowing for effective interaction with residents, their families, and other team members.
• A patient, empathetic nature with a true passion for serving the senior community.

Preferred Experience

• At least two years of relevant experience in geriatrics, assisted living, skilled nursing facilities (SNF), or home health environments, providing a strong foundation for delivering tailored care.
• Experience with interdisciplinary collaboration, enhancing treatment effectiveness through team efforts.

Compensation and Benefits

We offer a competitive salary in the range of $90,000 to $100,000 annually, commensurate with experience and qualifications. We also provide a comprehensive benefits package that includes:
