Work Environment

This role is based in a home care setting, allowing our Physical Therapists to engage directly with patients in their own environments. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical condition, needs, and rehabilitation goals.
• Develop and implement comprehensive treatment plans that incorporate tailored therapeutic exercises, hands-on techniques, and modalities to aid recovery.
• Utilize advanced methods within physical therapy, such as functional movement assessments and therapeutic exercise prescription, to promote mobility and decrease pain.
• Carefully document each patient's progress, treatment administration, and outcomes to ensure continuity and effectiveness of care.
• Regularly re-evaluate patient progress and make adjustments to treatment plans to reflect improvements or changing needs.
• Educate patients, their families, and caregivers on therapy techniques, assistive devices, and recovery processes to foster independence in their rehabilitation efforts.
• Stay up-to-date with the latest practices in physical therapy by participating in workshops, networking events, and professional development opportunities.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgeries, injuries, and chronic conditions. Each client presents unique challenges and opportunities for personalized care, allowing you to refine your therapeutic approach while seeing meaningful improvements in their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required for this role.
• Current state licensure in Physical Therapy is mandatory.
• Excellent verbal and written communication skills to effectively convey treatment plans and educate patients.
• Strong computer skills with proficiency in documentation and charting relevant patient data using electronic health records.
• Flexibility and agility in adapting to varying home environments and patient needs.

Preferred Experience

A minimum of two years of clinical experience as a Physical Therapist is preferred, although candidates with less experience but a strong educational background and enthusiasm for learning will also be considered. Compensation and Benefits
