Work Environment

This position is onsite, requiring Physical Therapists to travel to various patient homes throughout Gresham, Oregon. You will work in diverse home environments that necessitate a flexible and adaptive approach to therapy. Key Responsibilities

• Provide comprehensive physical therapy treatment focusing on preventing disabilities, relieving pain, and restoring function, as per individualized care plans prescribed by physicians.
• Conduct thorough evaluations of clients' functional abilities through diagnostic tests, assessing their muscle, nerve, and joint capabilities.
• Design and implement exercise programs, both manual and non-manual, to motivate patients and enhance their functional activities.
• Oversee the work of Physical Therapy Assistants to ensure an effective and coordinated approach to patient care, providing guidance and support where necessary.
• Evaluate patients' home environments, making recommendations to improve their independence and quality of life.
• Document clinical findings meticulously, developing initial evaluations, care plans, progress notes, and discharge summaries in compliance with regulatory standards.
• Participate as an integral member of the interdisciplinary team, engaging in case conferences to devise and refine comprehensive treatment strategies.
• Maintain regular communication and provide updates to client families and other healthcare professionals, facilitating a holistic approach to patient management.
• Ensure orders and documentation are submitted in a timely manner to uphold our standards of care.
• Provide education on the use and care of physical therapy devices, ensuring clients and their families are well-informed about their therapy programs.
• Engage in ongoing assessments and adjustments of care plans to reflect changes in clients’ conditions, maximizing their recovery potential.
• Complete any administrative responsibilities necessary for program compliance and operational success.

Patient Population / Client Focus

Qualifications

• A graduate of an accredited physical therapy program, holding a Bachelor’s Degree in Physical Therapy.

• Current and valid Physical Therapist licensure in the state of Oregon, ensuring compliance with legal practice requirements.
• Valid CPR certification and a driver's license with automobile insurance, enabling travel to patient locations.

Preferred Experience
