Work Environment

This position is based onsite, directly within patients' homes. The Physical Therapist will work collaboratively with other healthcare team members, including physicians, occupational therapists, and speech-language pathologists. You will have the flexibility of a supportive environment enabling you to deliver tailored therapy solutions in a comfortable space, eliminating the barriers that sometimes exist in clinical settings.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations, establishing individualized treatment plans that align with physician orders.
• Implement therapeutic interventions efficiently, utilizing techniques such as manual therapy, therapeutic exercise, gait training, and modalities to enhance recovery.
• Maintain accurate and timely documentation of patient care, including evaluations, treatment protocols, progress notes, and discharge summaries.
• Collaborate with interdisciplinary teams, sharing insights and recommendations to optimize patient care and outcomes while ensuring cohesive efforts across all health providers involved.
• Mentor and supervise Physical Therapy Assistants (PTAs) and support staff, providing guidance and oversight for patient care activities.
• Continuously assess the effectiveness of treatment interventions, adapting plans as necessary to meet patient goals and improve functional independence.

Patient Population / Client Focus

The ideal candidate will work with a diverse range of patients, including those with orthopedic injuries, neurological conditions, post-operative rehabilitation needs, and chronic illness. Your compassionate approach will be vital in empowering individuals from varying backgrounds and age groups to regain their functional abilities and enhance their mobility safely.

Qualifications

• Bachelor of Science degree in Physical Therapy from an accredited institution.
• Current licensure as a Physical Therapist in the State of Pennsylvania.
• Excellent verbal and written communication skills to effectively interact with patients, families, and interdisciplinary team members.
• Proficiency in utilizing electronic health record systems and other technological tools to monitor patient progress and document care.

Preferred Experience

• A minimum of one year of experience working in a physical therapy setting, preferably in home health.
• Familiarity with evidence-based practice guidelines and clinical protocols relevant to home care.
• Experience in working collaboratively within interdisciplinary teams to provide comprehensive patient care.

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications. In addition to the base salary, our benefits package includes:
