Work Environment

This position is primarily onsite, providing in-home therapy to clients across Silverthorne and surrounding areas. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, mobility, and functional capabilities, using standardized assessment tools and hands-on techniques.
• Develop individualized treatment plans that incorporate evidence-based practices tailored to each patient's unique health needs and rehabilitation goals.
• Implement therapeutic interventions, including manual techniques, exercise regimens, and modalities like ultrasound or electrical stimulation to alleviate pain, restore function, and improve mobility.
• Educate patients and their families on effective strategies for managing their conditions, including body mechanics, nutrition, home exercise programs, and activities of daily living.
• Maintain accurate and complete documentation of patient evaluations, treatment plans, and progress updates to ensure compliance with regulations and facility policies.
• Regularly assess treatment effectiveness and modify therapy interventions based on patient feedback and progress assessments.
• Actively participate in interdisciplinary team meetings to coordinate comprehensive care plans in the best interest of patients.

Our client base primarily includes adults and seniors dealing with various conditions such as post-surgical rehabilitation, musculoskeletal disorders, balance and gait deficits, and chronic pain management. Qualifications

• A bachelor's or master's degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist in Colorado.
• Board certification from the American Physical Therapy Association (APTA) is preferred.
• A minimum of 1 year of clinical experience in a relevant setting, with a strong understanding of various therapeutic modalities.
• Ability to effectively utilize current technologies and rehabilitation equipment used in home care.
• Strong communication skills to explain treatment plans clearly and confidently to patients and their families.
• Excellent problem-solving abilities to assess patient needs and adapt treatment plans accordingly.

Preferred Experience

• Previous experience in home health care settings.

Compensation and Benefits
