Hiring Now: Physical Therapist (Assisted Living Facility) | Orthopedic-Focused Multidisciplinary Practice

Salary Range: $90,000 – $100,000 per year

Location: Bogota, New Jersey

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

Are you a compassionate and skilled Physical Therapist looking to make a meaningful difference in the lives of older adults—without the rush of a traditional clinic?

Our established orthopedic outpatient practice in New Jersey is seeking a dedicated Physical Therapist to provide on-site therapy services at Assisted Living Facilities (ALFs) across the region. This unique opportunity blends the autonomy of homecare with the clinical depth of a multidisciplinary medical team, including Orthopedic MDs, PTs, OTs, and DCs working together for comprehensive, patient-first care.

Key Responsibilities:

  • Provide on-site therapy services to residents within affiliated assisted living communities
  • Perform evaluations and develop individualized, orthopedic-informed treatment plans
  • Coordinate with facility nursing staff, caregivers, and our internal team of MDs, OTs, and DCs
  • Educate residents and families on safe mobility, home exercise programs, and wellness
  • Leverage mobile EMR tools for efficient, HIPAA-compliant documentation
  • Act as a key liaison between the facility and our multidisciplinary clinic for referrals and follow-up

Qualifications:

  • Licensed Physical Therapist in the State of New Jersey (or eligible)
  • 1+ year of experience in outpatient, geriatrics, or ALF/SNF/homecare settings preferred
  • Strong interpersonal and communication skills
  • Clinical knowledge of orthopedic conditions common in aging adults
  • Comfort working autonomously in a residential environment
