We are searching for a talented Advanced Practice Practitioner to our Primary Care team.

Hours:  Part-time – 32 hours.

Shift: Day – no weekend, holiday or on call

Location:   Swansea, MA

• The Nurse Practitioner (NP) or Physician Assistant (PA), under the general supervision and direction of the Physician Lead/designee and Practice Manager will provide comprehensive primary care services to patients, functioning as an integral member of the healthcare team within a medical home model. This role emphasizes high-quality, patient centered, and evidence-based care in accordance with established clinical guidelines, regulatory requirements, and organizational policies.

• Physician Assistant: Bachelors Degree in a related field is required. 
• Graduate of an A.M.A.-approved Physician Associate /Assistant program is required.
• Nurse Practitioner: Master of Science Degree in Nursing is required.
• Experience in Primary Care is desirable.
• Physician Assistant: Current MA and/or RI Physician Assistant License is required based on practice location.
• Certification from a National accrediting body to practice as a Physician Assistant is required. Current BLS certification is required.
• Nurse Practitioner: Current MA and/or RI Registered Nurse and Nurse Practitioner License is required based on practice location.
• Certification from a National accrediting body to practice as a Nurse Practitioner is required. Current BLS certification is required.

USD $61.30 - USD $81.45 /Hr.
