Key Responsibilities

• Conduct thorough initial assessments of patients to identify their medical conditions, needs, and preferences, then collaborate with the multidisciplinary healthcare team to formulate customized care plans.
• Administer prescribed medications and treatments, monitoring their effectiveness while maintaining clear communication with physicians and updating care plans as necessary.
• Vigilantly monitor and document patients’ vital signs, symptoms, and changes in their medical history, ensuring precise and accurate records to facilitate seamless care continuity.
• Educate patients and their families on healthcare procedures, medication management, and chronic disease coping strategies, fostering informed decision-making and enhancing overall care quality.
• Assist physicians during medical procedures and transitions of care, ensuring patient comfort and safety throughout the process.
• Respond promptly to medical emergencies, providing critical care while maintaining a composed and supportive presence for patients and families in distress.
• Maintain detailed and accurate documentation of patient records, ensuring compliance with legal and organizational standards, while enhancing interdisciplinary communication.

Patient Population / Client Focus

In this role, you will primarily work with patients who are in the advanced stages of life-limiting illnesses. This includes individuals dealing with cancer, cardiovascular diseases, neurodegenerative disorders, and other terminal conditions. Your focus will be not only on maintaining comfort but also on supporting families through the emotional challenges of hospice care, fostering an atmosphere of empathy and open communication.

Qualifications

• Active Registered Nurse (RN) license, with eligibility to practice nursing in Tennessee.
• A Bachelor’s degree in Nursing (BSN) is preferred, demonstrating a commitment to professional excellence and advanced nursing practices.
• A minimum of one year of nursing experience, preferably within a hospital or hospice setting, showcasing your ability to work with complex patient needs effectively.
• Exceptional communication and interpersonal skills are essential for building rapport with patients, families, and the overarching healthcare team.
• Demonstrated ability to thrive in high-pressure and fast-paced environments, capable of managing multiple tasks efficiently while delivering quality care.
• Flexibility to work varied shifts, including weekends and holidays, as needed to meet the demands of our patients and organization.

Preferred Experience

• Prior clinical experience in hospice, palliative care, or geriatrics is highly desirable.
• Familiarity with electronic medical records (EMR) systems will enhance your workflow efficiency and accuracy.
• Crisis intervention training or similar certifications are a plus, highlighting your preparedness for emergencies.

Compensation and Benefits
